Catholics: What constitutes liturgical abuse and have you ever witnessed liturgical abuse if so what happened?

Well anything outside the liturgical guide is considered abuse. There are varying degrees from something minor like the priest for getting a line or forgetting to do something like the final blessing. I have seen priests invite teens to be present at the concentration and that was clearly against tradition.
